Welcome!
Below is some information to help you prepare for your first appointment. Please see the individual location page for information on location, parking, physician signature requirements, intake forms and billing.
For Your First Visit
Physician Signature
A script is not necessary for those going through self pay. If you would like to send your “Super Bill” to insurance for reimbursement, it is very likely that you will need a physician signature or script in order to do so.
What to Bring
Please bring a valid driver’s license or state ID to your first visit. Also bring any doctor scripts or notes that may be pertinent.
Clothing
Please bring or wear clothing that you are comfortable moving in that has the ability to expose the treatment area. Shorts and tank tops or sports bras are recommended.
Payment and Super Bill
I treat self pay only and I am not in contract with any insurance companies. Payment is due at the time of service and you can pay using cash, credit, check or your HSA if you have one. Though this may seem inconvenient, please see link below for the benefits and reasons for being a self pay clinic.
Though I do not take insurance, I can provide you with a receipt or “Super Bill” after each session that you can then submit to your insurance if you would like. Just call your insurance and let them know you are going to Physical Therapy and that I am not in network and ask what you need to do to submit for reimbursement.
